Job Description:
Summary
GENERAL SUMMARY:
Manages the day-to-day operations and personnel of the Influenza,
Foot Care, Cardiac Rehab, Peripheral Artery Clinic, Remote Patient
Monitoring, Wound and Ostomy, Ortho Navigators and the Diabetes
program both in the hospital and outpatient setting.
Operationalizes strategic and financial plans by monitoring,
evaluating and improving key organizational systems and processes.
Assists in developing and monitoring standards of services to
properly align human resources and patient needs.
PRINCIPAL JOB FUNCTIONS:
1. *Commits to the mission, vision, beliefs and consistently
demonstrates our core values.
2. *Monitors care environments to ensure compliance with federal,
state and local regulatory agencies; develops, revises and
communicates service policies and procedures.
3. *Interviews, hires, orients/trains, supervises and counsels
clinical staff; regularly rounds with employees and patients to
seek out feedback regarding satisfaction with processes,
efficiencies, equipment, environmental amenities; takes immediate
steps to rectify inadequacies or service complaints.
4. *Develops the service line plan for glycemic management
services, to include an interdisciplinary model of care that
involves pharmacy, nutrition services, nursing, care management and
laboratory services.
5. *Ensures the planning and implementation of education that
promotes professional competence and personal growth related to
glycemic management, including the Bryan hospital staff, physicians
and collaborative partners.
Works with Public Relations/ Marketing and Education Departments to
identify and provide appropriate educational offerings in the local
and regional community.
6. *Schedules staff based on patient census, staff qualifications
and competencies; anticipates staffing needs based on patient
referrals and takes steps to ensure promptness and continuity of
services.
7. *Provides leadership for the identification of and monitoring of
standards compliant for Glycemic management Programs.
8. *Develops capital and operational budgets based on strategic
initiatives, departmental priorities and financial status;
implements budget allowances and monitors compliance routinely;
communicates budgeting parameters to staff to ensure responsible
ordering and supply controls.
9. *Collaborates with physicians, staff and patients to design,
communicate and evaluate process improvement strategies based on
collaborative feedback and clinical best practices.
10. *Rounds with staff on a regular basis to ensure organizational
information is communicated, updates to policies and procedures are
discussed, work issues are openly addressed, and problems
professionally resolved.
11. Maintains supply and equipment inventories that meet patient
needs.
12. Ensures a patient care environment that is safe, clean,
clutter-free and focused on patient rehabilitation and
independence.
13. Develops growth strategies for all clinical services; develops
strategies to market and recommend other LifePointe services to
clinical patients to improve overall future health and
wellbeing.
14. Ensures billing compliance and initiates and reviews contracts
with BPN clinics.
15. Ensures credentialing of staff providing services at BPN
clinics.
16. Collaborates with BPN Providers, practice managers to establish
workflows.
17. Provides oversight to Diabetes Prevention Coordinator.
18. Provides oversight to Diabetes Quality Improvement coordinator,
reviews annual CQI, and supports certification process with the
ADA.
19. Updates order sets and standardizes processes appropriately
based on standard of care.
20. Ensures CMS compliance with multiple areas.
21. Maintains professional growth and development through seminars,
workshops, and professional affiliations to keep abreast of latest
trends in field of expertise.
22. Participates in meetings, committees and department projects as
assigned.
23. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
1. Knowledge of health, promotion and wellness philosophies, trends
and practices.
2. Knowledge of computer hardware equipment and software
applications relevant to work functions.
3. Knowledge of quality improvement measures, budgetary and
financial methods and practices.
4. Ability to establish and maintain effective working

families, physicians, business and industry representatives,
students and community groups.

balance personnel and financial resources to best meet
organizational, physician and patient needs.

EDUCATION AND EXPERIENCE:
Bachelors degree in nursing required. Masters degree preferred.
Current Registered Nurse licensure from the State of Nebraska or
approved compact state of residence as defined by the Nebraska
Nurse Practice Act. Minimum of five (5) years of clinical
experience in the health care industry, preferably focused on
glycemic management, required. Program development and management
experience preferred.
OTHER CREDENTIALS / CERTIFICATIONS:
Applicable certifications (i.e., Certified Diabetes Care and
Education Specialist (CDCES), Certified Diabetes Self-Management
Education (DSME) trainer, or nursing specialty certification)
preferred.
PHYSICAL REQUIREMENTS:
(Physical Requirements are based on federal criteria and assigned
by Human Resources upon review of the Principal Job Functions.)
(DOT) Characterized as medium work requiring exertion of 20 to 50
pounds of force occasionally, and/or 10 to 25 pounds of force
frequently, and/or a greater than negligible up to 10 pounds of
force constantly to move objects.
