Job Description:
A company is looking for a Remote Bilingual Certified Medical
Assistant Care Coordinator.
Responsibilities:
Conduct monthly phone calls with patients, practices, pharmacies,
etc. to ensure patients' needs are met between office visits
Create and revise personalized care plans for each patient in
coordination with practices and providers
Identify social determinants of health, gaps in care, and
eligibility for assistance or referral services for patients
Qualifications:
1-2 years healthcare experience as a Certified/Registered Medical
Assistant with active accreditation
Technology experience, preferably with multiple EMR/EHR systems and
Microsoft Office
Highly professional and compassionate phone demeanor with strong
critical thinking skills
Highly self-motivated, organized, and able to work
independently
Must have a home office set up for privacy and confidentiality with
fast and reliable internet
