Job Description:
We are looking for a motivated, reliable and enthusiastic MDS
Coordinator to join our AMAZING team!Must have a valid RN License
in Nebraska. MDS experience preferred.We currently have a Full Time
Opportunity Available with no routine 'on-call' requirementsDuties,
Responsibilities, and Essential FunctionsCompleting resident
assessment and care screening in order to comply with Federal and
State guidelines.Completes Minimum Data Set (MDS) upon resident
admission, quarterly, annually and with any significant change in
condition of resident.Visits and observes residents when completing
MDS. Coordinates with staff and other resources (family members,
physician, etc.) when providing care to the resident.Uses data
contained in resident medical record when completing MDS to include
medication and treatment sheets, Activities of Daily Living,
laboratory reports, and psychotropic medication
