CHI Health at Home in Lincoln has provided hospice services within
the local community for over 36 years. Focused on providing expert
care to people in the last stages of a life-limiting condition, our
compassionate team of caregivers provide the best quality of life
possible, through physical, spiritual, and emotional support that
respects the patients wishes and ensures peace. We are also
dedicated to each patients loved ones and family and strive to
create a sense of community and comfort through compassionate care
that extends to bereavement counseling and ongoing grief
purpose of the Hospice Home Health Aide is to perform personal
hygiene and grooming services, household care and provision of
nutrition. This position will work under the supervision of a
Registered Nurse, Physical Therapist, Occupational Therapist or
or assists patient with personal hygiene.
patient using the correct equipment, assists with ambulation, and
assists with active or performs passive range motion
meals considering dietary restrictions and client
basic household chores which may include/is not limited to: washing
dishes, changing bed linens and/or making beds, dust, sweep, mop,
vacuum, laundry and/or dispose of trash.
basic infection control guidelines to maintain a clean client
changes regarding patients physical/mental condition, completes and
submits documentation in a timely and appropriate
in case conferences, attends in-services, and/or special case
school graduate or equivalent.
- One year of Certified Nursing
Aide-Home Health Hospice.
obtain and maintain CPR or BLS certified.
completion of all competency testing.
