Behavioral Health Nursing Care Coordinator Location: Lincoln, Nebraska Department: Clinical Support Employment Type: Full-Time Minimum Experience: Entry-level Compensation: DOE Why do we need a Behavioral Health Nursing Care Coordinator? The Behavioral Health Nursing Care Coordinator serves as an extension of clinical staff and works to bridge the gap between the patient and their behavioral health provider. Most of the interaction is over the phone and in our EMR supporting the provider and triaging the patient's questions/needs. This role also works with pharmacy and other areas of the clinic to ensure that the patient is getting what they need and being compliant with the treatment plan. The Impact You'll Have You'll confer with families, agencies, and pharmacies regarding patient care Provides oversight and assists in determining the needs of the patient, including medication education to patients regarding medication effects and side effects Acts as a liaison between the medical provider and the patient for refill requests and other patient needs Need to Haves High school diploma Professional demeanor, ability to work amid interruptions, ability to handle patient complaints, excellent judgment in dealing with patients, care in protecting patient privacy and confidentiality Proficient verbal and written communication skills Ability to take initiative, exercise good judgment, maintain confidentiality, establish priorities, work independently, and ability to keep professional boundaries with patients. Individual must have the ability to work in a team environment.
