Must have candidates current name, no nicknames, or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable.Skills Checklist Completed within one (1) year of submittal for specialty where candidate is submittedTwo (2) References Two (2) professional references from work history occurring within the past three (3) years.
