worked, city and state, title, department, and dates employed from
the last 7 years. Agency names are not accepted as the employer.

Completed within one (1) year of submittal for specialty where

Meperidine, Tramadol. Dilute results require a retest.Physical
Completed within one (1) year of start date; required annually. May
be signed by Physician, NP, PA, DO, or DC.MMR Documentation Shown
by proof of two (2) MMR vaccinations or positive IGG titers. Will
accept one vaccine or decline form for low/equivocal titers,

and has 1 suite. They average 6-10 cases per week.Misc of 2 years
experience working in a Cath Lab setting. Requirements: BLS and
ACLS issued through American Heart Association are required. HCPs
are required to take MOAB if they don t already have CPI or MOAB.
The cost is $150.CPI certification refers to training provided by
the Crisis Prevention Institute, which teaches safe, evidence-based
methods for preventing and de-escalating challenging or aggressive
behaviors especially in healthcare, education, behavioral health,
and human-services settings.MOAB certification refers to training
provided by the MOAB Training International and stands for
Management of Aggressive Behavior. It is a professional program
that teaches staff how to prevent, recognize, and safely respond to
escalating or aggressive behavior in a wide range of environments
healthcare, schools, security, customer service, and public-facing
roles.Shift: Will be working 9-hour shifts (6:30A-4:00P) for total
of 36 hours per week. Call is required two weeknights each week and
every other weekend. Hours are not guaranteed the first week of
assignment, the last week, or when the department is closed (i.e.
holidays). Dress code: Hospital scrubs are provided. May utilize
their own skull cap but must be laundered by facility. Guaranteed
