Summary INTERNAL ONLY FOR CURRENT CRETE AREA MEDICAL CENTER
EMPLOYEES. ABOUT THE ROLE: Coordinates nursing care at Crete Area
Medical Center Outpatient Department. Coordinates the provision of
patient care consistent within standards, guidelines and
regulations, specific to all nursing care areas, projects day to
day staffing and anticipated staffing needs for next day and beyond
and communicates effectively. Participates as an RN in the
Outpatient setting and mentors and develops staff utilizing
critical thinking skills to achieve the highest quality patient
outcomes. Assists in meeting service excellence initiatives and
strategies as directed by leadership. YOUR ROLE WOULD ENCOMPASS:
Commits to the mission, vision, beliefs and consistently
demonstrates our core values. Collects subjective and objective
data and contributes to the assessment of the patient and documents
appropriately in the patient chart. Evaluates and communicates
patients progress to ordering provider. Facilitates patients
transition to next level of care if needed. Utilizes appropriate
resources to plan and provide nursing services that are safe,
effective and financially responsible. Collaborates with the
patient, family, providers and others in the provision of nursing
care. Assists in identifying teaching, learning and discharge needs
and communicates with providers, patient and family. Demonstrates
leadership in the professional practice setting and the profession.
Makes accurate and timely observation and documents changes in the
patients condition in the patient chart accurately and according to
policy. Performs a huddle with provider regarding identified
questions, individualized patient care and wellness needs. Performs
specialty clinic nursing skill set (blood transfusions,
chemotherapy administration, wound care, IV therapy, injections,
cardiology stress tests and aids with nuclear medicine testing).
Initiates assessment based on the patients presenting problems or
needs and the physicians medical diagnosis. Determines nursing
diagnosis and priorities for care with special consideration given
to patient teaching, goal setting, psychosocial well-being and
functional status. Assesses tolerance of activity levels and
adjusts program as determined by patients ability. Addresses and
applies interpretation of diagnostic/monitoring data to the plan of
care. Appropriately interacts with patients/families from a variety
of developmental and sociocultural backgrounds; seeks interpreter
assistance as appropriate. Provides information to patients and
families, which reduces anxiety and conveys an attitude of
acceptance, sensitivity and caring. Initiates communication with
health care team members regarding treatment planning; informs
appropriate health care team members (including physicians) of
patients condition/rehab status. Completes clinical/non-clinical
assignments based on departmental workload and the qualifications
and competencies of self and other staff members. Assesses
individual educational needs, barriers for learning and learning
styles and provides appropriate learning experiences; reviews,
develops and provides educational materials appropriate to patient
needs. Provides education to patient and significant others
regarding disease entities, risk factor management and behavior
modification and promotes primary, secondary and tertiary
prevention. Acts as a department resource and customer service
champion for staff, providers, other team members. Acts as champion
ambulatory clinician in Epic Epic Amb SuperUser for Specialty
Clinic and OP Nursing, including Epic Therapy Plans. May be asked
to help other organizational departments in their use. Schedules
referrals department work queue and assists with Recall List.
Administers medication according to policy and provider orders.
Performs cardiolyte and stress tests and coordinates scheduling of
these tests. Maintains a neat, organized, and safe environment for
the patients, visitors, and CAMC team. Assists with staff schedules
for the department. Acts as a liaison between department and
specialists offices. Helps coordinate day-to-day activities within
the department including nurse appointments, provider schedules,
etc. Reviews Policies and Procedures with department team members
and make appropriate updates with Dept Manager Assists with
department goals (monthly reports, department goal setting,
implementation tracking). Assists with coordination and
communication on assigned tasks and projects. Maintains
professional growth and development through seminars, workshops,
and professional affiliations to keep abreast of latest trends in
field of expertise. Participates in meetings, committees and
department projects as assigned. YOUR EXPERIENCE INCLUDES (PROVEN
PERFORMANCE IN): Graduation from an accredited professional nursing
program required. BSN preferred. Current Registered Nurse License
in the State of Nebraska required or approved compact state of
residence as defined by the Nebraska Nurse Practice Act required.
Must provide evidence of certification and/or instruction for
practice area as required by State or Federal
standards/regulations. Minimum of two (2) years of experience
within the outpatient nursing setting. Current Basic Life Support
(BLS) certification required. Bryan Health recognizes American
Heart Association (for healthcare professionals), American Red
Cross (for healthcare professionals) and the Military Training
Network. Advanced Cardiac Life Support (ACLS) required. Bryan
Health Chemotherapy Education required prior to administering
chemotherapy independently.
