Bluffs, IA 51501.The schedule for this position is Monday- Friday
8am-4:30pm with incentivized on call every 6-7 weeks!Pay will be

Duties & Skills Required:What You Will DoThe Clinic RN is essential
in delivering the healthcare components of the established care
plan at Immanuel Pathways. This role involves assessing and
triaging the medical needs of participants attending the Day Center
or calling into the clinic, ensuring timely and appropriate care.
The Clinic RN also performs a variety of nursing tasks, effectively
communicates participant care information to the Primary Care Team
and may represent nursing in IDT meetings as needed. This position
requires a compassionate, detail-oriented professional committed to
providing high-quality care to elderly participants, fostering
collaboration with interdisciplinary teams, and promoting
participants' independence. The Clinic RN actively supports and
upholds Immanuels Mission and CHRIST Promises.Key AreasKey
Responsibilities and Duties of the JobParticipant CareParticipates
as an active member of the Primary Care Team.Triage the medical
needs of participants, determining appropriate levels of
care.Conduct nursing assessments and administer treatments as
necessary.Provide intravenous (IV) therapy and phlebotomy as
necessary.Provide wound care management, including assessment,
treatment, and monitoring of wounds to promote healing and prevent
complications.Communicate and collaborate with participants,
healthcare professionals, IDT, and caregivers regarding participant
care.Monitor participant response to treatment and adjust care
plans accordingly.Document all assessments, interventions, and
participant responses in compliance with Immanuel Pathways
standards.Revises participant care plan in collaboration of the
IDT.Educate participants, families, and caregivers on care,
medications, and treatments, ensuring documentation of all
educational efforts.Participate in department and center meetings
and contributes to quality and compliance initiatives.Support
infection control standards and report infections per surveillance
guidelines.Participate in on-call rotation and adheres to the
on-call standards and documentation.May assume responsibilities of
absent team members as needed.May on rare occasion visit and assess
participants in their homes.May conduct telehealth visits with
participants or caregivers.Act as lead in the clinic in the absence
of the Clinic Nurse Supervisor.Ensure adherence to OSHA safety
guidelines and Emergency Preparedness Plans.Maintains a safe
environment by using the proper lifting and transferring
techniques.Maintain confidentiality and protect participant
privacy.ComplianceIn conjunction with the Clinic Nurse Supervisor,
implements the Infection Control Plan to ensure a safe and sanitary
environment.Responsible for tracking and maintaining post exposure
evaluation and participant surveillance reports.Follows all
Immanuel Pathways policies and Procedures and OSHA safety
guidelines.Protects privacy and maintains confidentiality of all
company procedures, results and information about employees,
participants, clients and families.Participates in and supports
Quality Improvement initiatives.Maintains safe working environment.
Follows Immanuel Pathways Safety policies and procedures.Implements
Exposure Control Plan.Complies with Emergency Preparedness
Plan.Uses proper lifting and push/pull techniques, uses gait belt
assistance on participant transfers as needed.Provides first aid
treatments as needed.Acts within the scope of his or her authority
to practice.OtherEngage in continuing education and maintain
professional affiliations and required certifications.Perform
additional duties as requested or required.Skills &
RequirementsEducation-Bachelors degree in nursing preferred;
professional nursing school graduate required.Current RN license in
the respective state of employment required (preferably compact
state licensure for NE and IA).Current Cardiopulmonary
Resuscitation (CPR) certification required.Wound, Ostomy, and
Continence Nurse (WOCN) certification is a plus; training may be
provided during employment.Experience-Minimum of two (2) years of
nursing experience in a hospital, nursing home, or community-based
setting, preferably in geriatric care.Minimum of one (1) year of
experience working with the frail or elderly population, or
completion of job-specific training related to working with the
elderly population within six months of hire.Other
Requirements-Legally authorized and licensed to practice as an RN
in the respective state of employment.Must act within the scope of
authority as an RN in the respective state.Maintains professional
affiliations and required licensure and certifications.Must agree
to abide by the philosophy, practices, and protocols of the PACE
organization.Competency in job-specific nursing responsibilities
must be met before assuming participant care duties.May be assigned
to multiple PACE Pathways sites as required.Medical clearance for
communicable diseases and up-to-date immunizations required before
direct participant contact.Valid drivers license, proof of
