Job Description:

PRINCIPAL JOB FUNCTIONS:Client ServicesProvides individual, dyadic, or family therapy to clients as assigned.May develop and manage education and/or mental health groups (general or specific topics).Acts as case manager for their assigned clients.Assists in the authorization of services.Documents and manages the clients case file.Completes assessment, treatment planning, and the documentation of services provided to the client.Makes collateral contacts as needed for clients and obtains releases of information for those contacts.Refers clients to internal or external services to address clients needs or as requested by the client.Terminates service when it is in the best interest of the client or at clients request.Record Keeping/Case DocumentationThe case record is the responsibility of the therapist.Files must be kept up to date, document the reason(s) for continued treatment, and in good order.Therapist assesses client needs and establishes a written treatment plan.Therapist documents the occurrence of on-going service, documents progress or changes to the service plan, and assists in the development of plans for discharge.Services are documented in the file and documentation is submitted on time.Community RelationshipsTherapist recognizes the importance of contacts with the community and consistently maintains a professionalism in interacting with others.Relationships with other professionals and systems will be maintained to guide the opportunity for clients to self-determination in a strength-based, supportive environment.Therapist will involve themselves in community activities/organizations that promote the agency, its staff, and the provision of needed services for clients.Agency Involvement/Professional DevelopmentTakes responsibility in continued learning about changes in program/agency for their professional growth.Attends required meetings.Uses opportunities offered by the agency to strengthen agency or professional knowledge/skills.Participates in agency Performance and Quality Improvement (PQI) activities.Interpersonal RelationshipsContacts by the therapist will conform to, and be guided by, the ethical standards of the profession.Therapist will strive in their work to interact in a way that is supportive of clients, the agency, and the profession.Cultural SensitivityEmbraces and displays sensitivity relative to the cultural and socioeconomic characteristics of the agencys service population.Mission and Core ValuesCompletes work assignments/expectations within the agencys mission of Helping Families Thrive and embodies the agencys core values by being family-focused, adaptable, passionate, collaborative, accountable, and preventative.Performs other duties as assigned.REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:Able to build relationships with clients, co-workers, and professionals in the community. Must possess effective communication skills. Able to work independently with guidance and support from supervisor. Must be flexible, responsible, and reliable.EDUCATION AND EXPERIENCE:Masters degree in counseling, social work, marriage and family therapy, community counseling, or other field of work. Six months to one year of previous experience.OTHER CREDENTIALS / CERTIFICATIONS:PHYSICAL REQUIREMENTS:Characterized as medium work requiring exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or a greater than negligible up to 10 pounds of force constantly to move objects.
